			<title><![CDATA[MR Imaging of Temporal Lobe Epilepsy]]></title>

			<description><![CDATA[H MRS) has been proposed as a lateralizing method for the presurgical evaluation of patients with medically intractable temporal lobe epilepsy (TLE). Studies have shown correlations between temporal lobe (TL) NAA and seizure frequency and TL NAA/Cr and the duration of epilepsy in patients with TLE. This latter finding may suggest that progressive neuronal dysfunction may occur in both temporal lobes in patients with TLE even when the seizures originate in only one temporal lobe. We analyzed our data in an attempt to find a possible correlation between extension of neuronal dysfunction based on NAA measures and duration of epilepsy.
The term hippocampal sclerosis was originally used to describe a shrunken and hardened hippocampus which histologically displayed neuronal loss and glial proliferation. These alterations are mainly located in the hilus of the dentate gyrus and in the CA1 and CA3 pyramidal cell layers but all hippocampal regions may show neuronal cell loss to varying degrees. A number of morphologic and cytochemical findings are associated with mesial temporal sclerosis especially within the dentate gyrus. These changes include selective loss of inhibitory interneurons abnormal sprouting of axons reorganization of neural transmitter receptors alterations in <a href='http://second.wordsblogs.com/'>second</a> messenger systems and hyperexcitability of the granule cells. Extrahippocampal pathology is also found at other temporal lobe structures. Frequent extrahippocampal pathology affects the amygdala first seen with neuronal cell loss and gliosis in the laterobasal complex. Surgical removal of this epileptogenic area can be curative or provide significant reduction in seizure frequency in the majority of individuals. Magnetic resonance imaging (MRI) is highly sensitive in detecting and locating mesial temporal sclerosis when a correct MRI temporal lobe protocol is used. The most <a href='http://important.wordsblogs.com/'>important</a> MRI findings atrophy and abnormal T2 signal allow us to detect mesial temporal sclerosis in the majority of the cases. Secondary MRI findings help in the diagnosis and lateralization of mesial temporal sclerosis in patients with subtle primary findings and in cases of bilateral hippocampal abnormalities. The development of advanced magnetic resonance (MR) techniques such as functional MR diffusion or transference of magnetization will lead to greater understanding of this pathology and will improve our diagnostic capacity.
The incidence and structural basis of nonlesional medial temporal lobe epilepsy (MTLE) without hippocampal atrophy (HA) is not known. Reports suggest that 5&#x2013;15% of patients with hippocampal sclerosis (HS) do not have HA. We reviewed 57 patients with MTLE who underwent magnetic resonance imaging (MRI) volumetric studies anteromedial temporal lobectomy (TL) and quantitative cell counts of the resected hippocampus and who had at least 1 year of surgical follow-up. We <a href='http://compared.wordblogs.net/'>compared</a> demographic pathologic and outcome data among patients with and without HA. Ten of the 57 patients did not have HA. Nine of the 10 patients had <a href='http://depth.wordsblogs.com/'>depth</a> EEG for localization of seizure onset because of discordance or insufficient localization. Five patients had pathologic evidence of HS. Longer duration of epilepsy correlated inversely with the presence of HA. Outcome was not significantly different between patients with and without HA. We conclude that the incidence of MTLE and HS without HA is 9%. Except for duration of epilepsy there were no differences in the clinical syndrome or surgical outcome of patients with and without HA.
The G1465A polymorphism in the gene of the GABA type B receptor subunit 1 (GABABR1) has been linked to the risk for temporal lobe epilepsy (TLE). However six replication studies did not show significant association between the G1465A GABABR1 gene variant and TLE. The authors examined this association in a sample of 102 patients with mesial TLE with hippocampal sclerosis (MTLE-HS) and 71 controls. The genotype distribution varied significantly between patients and controls. Heterozygous <a href='http://carriers.wordblogs.net/'>carriers</a> of the A-allele had a 10-fold <a href='http://increase.mortgageblogs.net/'>increase</a> in risk for MTLE-HS (OR 10.01; 95% CI 3.98&#x2013;25.18 p&#xa0;=&#xa0;3.788E&#x2212;08).

			<description><![CDATA[Sentinel lymph node (SLN) procedure is now a widely accepted method of LN staging in selected invasive breast <a href='http://cancers.wordblogs.net/'>cancers</a> (unifocal size [less than or equal to] 2 cm clinically N0 without previous treatment). end axillary clearance is no longer needed if the SLN is negative. However the oncological safety of this procedure remains to be addressed in randomized clinical trials. One main pitfall is the failure to visualize SLN resulting in incorrect tumor staging leading to suboptimal treatment or axillary recurrence. Operative gamma cameras have therefore been developed to optimize the SLN visualization and the quality control of surgery.

			<description><![CDATA[I'm going to be up all the cliches and metaphors I can find <a href='http://about.obscureblogs.com/'>about</a> rain. (If you undergo a favorite expression about rain please e-mail it to me. Consider it a birthday greeting.)
When I was a little girl my family lived in Hoquiam a small town two hours or so southwest of Seattle on the glide. It rains much <a href='http://more.wordsblogs.com/'>more</a> in Hoquiam than it does in Seattle and what I remember <a href='http://from.choiceblogs.com/'>from</a> having birthday parties in November is that my presents were always wet!
They would get wet on the trip between car and accommodate. soften gift paper. Bedraggled bows. It's a <a href='http://funny.wordblogs.net/'>funny</a> memory to hold onto after all these years. But that's what I remember. 
CelebratingI went to the women's bathhouse with my friends Monica and Laurie last night to celebrate my birthday. That was wonderful. We ate Korean tofu stew at the bathhouse's little restaurant and soaked and scrubbed and steamed. Lay in the Salt dwell (140 degrees) for awhile letting the impurities flow out of my body in my sweat... (Or so they say. I just know it feels incredible.)
Today my actual birthday. I'm not doing much except talking on the phone with friends and family who call to wish me a happy birthday and answering birthday e-mails from friends in Europe and elsewhere. Older Son just called to express me he loved me which was <a href='http://sweet.wordsblogs.com/'>sweet</a> since he and I undergo been battling lately. 
Tomorrow I'll have my birthday dinner. One friend is baking the cake; another is bringing the booze. And I'm making an Italian meal--eggplant plus salad and garlic bread. I think I will make the cover from scratch and check in the garden to see if my garlic is big enough to be harvested. 
My garden is pretty change intensity alter now but I still undergo lots of onions garlic and chives to last me through the winter. Seattle's winters are mild enough that all of these should just act on growing. Fresh onions and garlic undergo so much more flavor than those bought at the store. 
PainMy broken rib is <a href='http://really.musicalblogs.com/'>really</a> hurting and I undergo it strapped up with a rib brace that I bought at the drugstore yesterday. Also took pain meds and I'm going to ice it off and on today. Monday I'm going in to see my <a href='http://radiation.musicalblogs.com/'>radiation</a> oncologist and it looks like I'll be doing another series of radiation treatments to the rib. 
Also <a href='http://decided.wordblogs.net/'>decided</a> during this latest move of activity on the cancer front that I do want to see Dr. Livingston in Tucson fairly soon. So I'm going to try to make an appointment with him for the first week of January. 2008. That will be the sixth anniversary of my metastasis. 
Dr. Livingston is still the one I trust to take a look at EVERYTHING that is going on with me and give me his advice. What do I get out of this? <a href='http://peace.prescriptionblogs.com/'>Peace</a> of object. And probably a few more years of life. 
The Number 53My father died of pancreatic cancer at age 53. (I was 21 at the time.) So ever since my cancer diagnosis <a href='http://nine.wordsblogs.com/'>nine</a> years ago now. 53 has been hanging out there in front of me--Would I live that desire? Would I die at 53 as my create did? 
When my older brother was diagnosed with metastatic colon cancer at age 52. I thought that HE would probably die at 53. (He lived to be 54 and died this past September.)
But now that I've hit 53. I evaluate that I will live to be at least 55. Regular readers of my blog may remember that I have a Two Year Plan for my life. That is. I intend in two-year increments. I just recently reset the <a href='http://clock.wordsblogs.com/'>clock</a> and have made some decisions about the next two years of my life which will carry me to age 55. 
These include a trip to Hawaii in February (an annual move with my friend Monica. This will be our third Hawaii trip together); the Cancer Bloggers Reunion in Seattle in July; and a long visit to Japan in October to flee from pink ribbon madness. 
And I'm planning next year's garden. I want to lay more carrots potatoes and tomatoes because I didn't undergo enough of those. Maybe more green beans too. The beans were delicious this year. And eggplant--I didn't lay any this year and I like it especially the Asian varieties. 
I have a fig tree still in its pot on the front porch that needs to go into the back garden next to the Japanese nashi (pear-apple) the grape vines and the berries. When the come down stops. I'll act care of that. And mulch the roots come up so that it will survive the winter. <br>
